Advance Care Planning (ACP) helps ensure your patients’ health care and end-of-life wishes are known to you and their other health care providers, their caregivers and loved ones before a serious illness or health crisis.  This session, led by the Kansas Healthcare Collaborative, discussed methods for normalizing and engaging in the ACP process and how to complete ACP documents, like a living will.  Attendees also learned about the role of the patient and those appointed to act on the patient’s behalf, as well as how the ACP process is reflected in Kansas and Missouri law.
