Join hospitals across the United States reducing avoidable utilization through a new, federally sponsored program that complements the focus of Hospital Improvement Innovation Networks (HIINs) and many hospital or health system initiatives.
The Centers for Medicare & Medicaid Services (CMS) funded Health Quality Innovators (HQI) to support hospitals over the next five years. This initiative, which HQI is calling the Health Quality Innovation Network (HQIN), helps hospitals strengthen ties with local providers and community organizations for a comprehensive approach to managing care transitions. The Carolinas Center for Medical Excellence (CCME) and other local improvement partners support assist hospitals in South Carolina and Kansas.
HQIN membership is no-cost, collaborative and includes learning from high-performing hospitals that share best practices and helpful tips. Benefits include data reports and analyses to help you track your performance – and expert consultation to help you act on your results. You also will have access to improvement tools, resources and quick-hitting, bite size training sessions.
